In the late 1970s and early 1980s, Madsen embarked on his professional acting career in Hollywood. He made a significant mark by playing Lieutenant Steve Phelps in the 1983 film WarGames. Later, he appeared in movies like The Natural (1984), Kill Me Again (1989), and Thelma & Louise (1991), among others.

After the success of Reservoir Dogs, his fame spread throughout households, leading to numerous film appearances such as Free Willy (1993), Wyatt Earp (1994), Donnie Brasco (1997), Die Another Day (2002), Sin City (2005), Scary Movie 4 (2006), Vice (2008), and many more. Furthermore, he lent his voice to Maugrim in The Chronicles of Narnia (2005).
